Managing your condition at home
Overview
Remote monitoring helps us keep track of how you are feeling. Using your smartphone, you can answer a few questions each month to help us give you the right care at the right time.
We’ll use your responses to:
- monitor changes to how you are feeling
- talk to you about your experience when you come for an appointment
- make changes to your treatment or care plan
- arrange treatment when you need it (for example, bringing your appointment forward)
- send you information to help you manage symptoms at home

We are using remote monitoring across several NHS Trusts and departments.
For rheumatology patients
We use remote monitoring to help people manage:
- Rheumatoid Arthritis (RA)
- Spondyloarthritis (SpA)
- Psoriatic Arthritis (PsA)

Important: If you need support now
You don’t need to wait for your monthly monitoring form if you need help.
Call your hospital if you:
- are ‘in flare’ or your symptoms get worse
- need medical advice about your condition
- need to book or change an appointment
- want to check your appointment date or time
- need to find out when you’ll get your prescription
- want to check the status of a referral
Always call the hospital you usually visit. Teams in other hospitals will not be able to access details of your appointments, prescriptions or your health records.
